1. GENERAL PROVISIONS

The website „Centrum Św. Łukasza Krzysztof Majdyło Sp. k.” (hereinafter referred to as the „St. Łukasz website”) available at www.swietylukasz.pl is operated by the company Centrum Św. Łukasza Krzysztof Majdyło Sp. k. with its registered office at ul. Narwickiej 18, 80-557 Gdańsk, NIP: 9571074473, email address: marketing@swietylukasz.pl, hereinafter referred to as the „Service Provider.”

These Regulations are directed at both consumers and non-consumers using online payment services (except for provisions intended exclusively for consumers).

The administrator of the personal data of individuals processed in connection with the provisions of these Regulations is the aforementioned Service Provider. The personal data is processed for the purposes, scope, and under the principles specified in the privacy policy published on the website www.swietylukasz.pl. The provision of personal data is voluntary, but necessary to proceed with the provision of services. Each natural person whose data is processed by the Service Provider has the right to access, update, and request the deletion of their data under the terms specified in Regulation (EU) 2016/679 of the European Parliament and the Council of 27 April 2016 on the protection of natural persons concerning the processing of personal data and the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (Journal of Laws of the EU. L 2016, No. 119, p. 1, as amended).

Please be informed that the services provided under these Regulations include healthcare services provided by healthcare professionals to patients regarding the assessment, maintenance, or improvement of their health status, in accordance with legal provisions, excluding consumer rights regulations under Art. 3 sec. 1 point 7 of the Act of 30 May 2014 on consumer rights (i.e., Journal of Laws of 2020, item 287, as amended). The provisions of Art. 12-14, Art. 15 sec. 1, Art. 18, Art. 21-24, Art. 26-37 of this Act do not apply to these services. Consequently, due to the nature of the services, the consumer does not have the right to withdraw from the contract under Art. 38 point 1, 3-6, and 8 of this Act.

By accepting these Regulations, the User declares that they have read their content, agree to their provisions, and are aware of the loss of the right to withdraw from the contract in the case of entering into an agreement for the provision of healthcare services.

 

2. TERMS OF ELECTRONIC SERVICES

a/ Using the Service Provider’s services requires reading and accepting the provisions of these Regulations, as well as meeting the technical requirements.

b/ Users are obliged to use the services in a manner consistent with applicable laws, these Regulations, and generally accepted principles of using the internet.

c/ The minimum technical requirements for using the Service Provider’s online services are:

[1] A computer, laptop, or another device with internet access;

[2] Access to an active email account.

[3] Web browser requirements:

    • Mozilla Firefox version 17.0 or later
    • Internet Explorer version 10.0 or later
    • Opera version 12.0 or later
    • Google Chrome version 23.0 or later
    • Safari version 5.0 or later

[4] Recommended minimum screen resolution:

    • 1024×768

[5] Enabling cookies and JavaScript in the web browser is required.

The Service Provider’s website offers the following electronic service: the ability to make an online payment for a scheduled visit to Centrum Św. Łukasza.

To use the service, follow these steps:

  1. Fill out and submit the „Quick Contact” form on the Centrum Św. Łukasza website. The form requires the patient’s details, including their name and/or surname, phone number, and optionally an email address. It is also possible to schedule an appointment by calling the provided phone numbers or sending an email to:

  2. Next, the payment for the visit must be made. The client has the following payment options:
    • Online payment method: available on the Centrum Św. Łukasza website via the Dotpay service. The user will be redirected to Dotpay.pl after clicking the appropriate link on the Centrum Św. Łukasza website.
    • Bank transfer: the payment should be made to the Service Provider’s account at ING Bank Śląski:

      • Payment title: „Consultation visit”
      • Payment confirmation should be sent to:

        • info@swietylukasz.pl

d/ Payment Terms:

  • If payment is made via Przelewy24.pl or a traditional bank transfer, the user must complete the payment on the same day they submit the consultation request form. The payment date is considered the day the user’s bank account is charged.
  • If payment via Przelewy24.pl or bank transfer, along with the payment confirmation, is not received within three days before the visit, the appointment will be automatically canceled.

3.USER OBLIGATIONS

The user is obligated to use the Centrum Św. Łukasza website in compliance with the law and good practices, respecting personal rights, copyright, and intellectual property rights of the Service Provider and third parties. The user must provide truthful data and is prohibited from submitting content of an unlawful nature.

4.SERVICE COMPLAINTS

The Service Provider is obligated to provide services to the user in accordance with the regulations available at the Service Provider’s office and on the Centrum Św. Łukasza website.

If the service is not provided in accordance with the contract, regulations, or legal provisions, the user has the right to submit a complaint. A complaint may be submitted:
